I think the chorus of 'I am not afraid to keep on living' was meant to be ironic as they were the 'Famous Last Words' of the patient who had just looked back on his life and decided it was worth going on and then he died. I'm pretty sure the album is not meant to be depressing.

Just got back from seeing Gallows at Leadmill. They are amazing seriously you have to see these guys plus since they were on the small stage Frank Carter ran around the crowd(he also landed on me).
